ABSTRACT

The outbreak of the recent Covid-19 pandemic changed many aspects of our daily life, such as the constant wearing of face masks as protection from virus transmission risks. Furthermore, it exposed the healthcare system's fragilities, showing the urgent need to design a more inclusive model that takes into account possible future emergencies, together with population's aging and new severe pathologies. In this framework, face masks can be both a physical barrier against viruses and, at the same time, a telemedical diagnostic tool. In this paper, we propose a low-cost, 3D-printed face mask able to protect the wearer from virus transmission, thanks to internal FFP2 filters, and to monitor the air quality (temperature, humidity, CO2) inside the mask. Acquired data are automatically transmitted to a web terminal, thanks to sensors and electronics embedded in the mask. Our preliminary results encourage more efforts in these regards, towards rapid, inexpensive and smart ways to integrate more sensors into the mask's breathing zone in order to use the patient's breath as a fingerprint for various diseases. © The Author(s), under exclusive license to Springer Nature Switzerland AG 2023.

ABSTRACT

We report a clinical case of an extremely rare neuroendocrine tumor of the right middle ear (MeNET) that recurred after 13 years with a local extension into the right temporal fossa. In the current medical literature, there are approximately 150 cases of MeNETs and even fewer cases with more than 10 years of follow-up, recurrence, and intracranial tumor progression. Therefore, we believe that this paper can make an important contribution to the existing and future knowledge about this disease. The purpose of this article is to present our experience in treating such a rare neoplasm in a 35-year-old woman. The patient initially complained of worsening hearing in her right ear over the past year. The final diagnosis was made based on the findings of computed tomography (CT), magnetic resonance imaging (MRI), and histological and immunohistochemical evaluation of excisional biopsies of the original and recurrent tumors. The primary tumor masses were removed with clear resection margins, and the ossicular chain was reconstructed. The patient has been monitored clinically and radiologically with temporal bone CTs every year and MRIs three times in general since then. A postoperative audiogram showed remaining mixed hearing loss in the right ear that eventually worsened as the tumor grew. Tumor recurrence and progression after 156 months (13 years) were seen on CT and MRI, requiring further treatment. After resection of the recurrent tumor, paresis of the right facial nerve developed, which was treated with dexamethasone. The surgical treatment caused the initial symptoms to disappear, but the facial nerve paresis persisted with mild functional improvement. The patient is not receiving adjuvant radiotherapy and is being monitored closely because the tumor may recur in the future.

ABSTRACT

Mucosal-associated lymphoid tissue lymphoma is extremely rare due to the scarcity of lymphoid tissue in the trachea. To date, approximately 20 cases of tracheal mucosa-associated lymphoid tissue lymphoma have been reported. This case report presents a primary tracheal extranodal marginal zone lymphoma case detected incidentally during the coronavirus disease-2019 screening.

ABSTRACT

The study presents the results of a numerical analysis of the effectiveness of the use of personal protective equipment of various designs on the spread of pollutants marked with CO2 emitted during human breathing. In the study of 3D geometry the upper part of the human torso and head was developed. The simulated person was supplied with different personal protective equipment covering the human face (PPE). Two types of face shields worn at a different distance from the face and one fabric face mask was analysed. The reference geometry with no personal protective equipment was also analysed. Transient calculation with full breathing model including breath-in and breath-out and species transport were simulated. The results showed that different PPE generates different airflow patterns in the vicinity of the human face. The most efficient in reducing infection risk is by wearing a face mask or face shields at a small distance from the face, as they most effectively reduce CO2 concentration in the surrounding air. However, they also increase the re-inhalation risk of high CO2 concentration which affects human well-being.

Amidst the global health emergency, when couples of academicians are devoted to pursuing their research linked with COVID-19, this present chapter is purposively concerned with the lesser-highlighted issue of customarily categorized livelihoods scenario, on a spatial basis, in one of the nations of Global South. The prime objective of the present section is to find out the fundamental fashion of regional deviation as well as the concentration of livelihood and future suggestions for suitable policy proposals. While this volume is systematically based on secondary datasets from recognized sources and the methods are being adopted after judiciary modification of established modus operandi like ‘crop combination', ‘location quotient', ‘crop diversification', and ‘GDP geographical area ratio'. On the other side, for the overall ‘livelihood zone map' (LZM), the standard ‘Z'-score method, and GIS mapping tool have been used. Although the regional data-oriented outcome is much voluminous, in a nutshell, it can be affirmed that in Goa, Delhi (NCR), West Bengal, and Manipur, the quality of livelihood condition is well, while among rest of the Indian states and union territories, the status is below the desired level. ABSTRACT

This study contributes to a better understanding of the airborne transmission risks in multizone, mechanically ventilated buildings and how to reduce infection risk. A novel modeling approach combining the Wells-Riley and the US National Institute of Standards and Technology (NIST) CONTAM models was applied to a multizone whole building to simulate exposure and assess the effectiveness of different mitigation measures. ABSTRACT

Urbanization of coastal areas worldwide has increased due to an increase in the global population. The production of sustainable aquaculture is greatly impacted by a surge of this urbanization. In certain countries, particularly for individuals with more limited space in metropolitan areas, such as along Johor's coastal area, aquaculture might well be a good strategy to maintain food availability (continuous production plus high-quality meals). Consequently, the adoption of aquaculture along the Johor's coastal area has lead to Harmful Algal Blooms (HAB). This paper examines the evolution of the aquaculture industry of Malaysian Johor coastal areas in relation to HABs. In addition to HABs, the aforementioned metropolitan regions confront diverse economic and geographical obstacles when attempting to increase their aquaculture production sustainably. Those problems are therefore addressed using a variety of operations as well as surveillance techniques in this brief overview. Lockdowns and border prohibitions caused by the continuous COVID-19 infection have had a global impact. These logistical difficulties in the seafood industry have increased dependency on imported supplies. It is suggested that international decision- making, supervision, and knowledge exchange can successfully solve the challenges urbanized areas have in ensuring sustainable food security through the evolution within the aquaculture sector.

ABSTRACT

Introduction: Mucosa-associated lymphoid tissue (MALT) lymphomas are extra-nodal marginal zone B-cell lymphomas, most commonly found in the stomach, associated with Helicobacter pylori infections, and generally not linked with inflammatory bowel disease. Rectal MALT lymphoma is very uncommon and often associated with painful defecation, change in bowel habits, or rectal pressure/prolapse. Here, we present a rare case of an asymptomatic female with ulcerative colitis (UC) found to have benign-appearing rectal polyps during a routine screening colonoscopy. Case Description/Methods: The patient is a 56-year-old female with a history of left-sided UC, diagnosed in 1993, with one flare after receiving the 2nd dose of the Moderna COVID-19 vaccine, taking oral Olsalazine 500 mg twice daily, low-dose Prednisone, and mesalamine suppositories as needed presenting for screening colonoscopy. The patient was asymptomatic, citing regular non-bloody bowel movements and normal stool consistency. Colonoscopy revealed two 7 mm sessile, non-bleeding rectal polyps, surrounded by congested, erythematous, friable, and ulcerated mucosa in the rectosigmoid colon. Cold forceps biopsies were taken. Hematopathology evaluation of the routine colon biopsy samples revealed chronic nonspecific colitis while pathology of the rectal polyps showed marked lymphoplasmacytic infiltrate and extra-nodal marginal zone lymphoma of MALT. Ancillary studies, immunohistochemistry, and molecular studies for B-cell gene rearrangement confirmed extra-nodal marginal zone lymphoma of MALT with prominent plasmacytic differentiation. The patient was informed and close follow-up in Gastroenterology clinic was arranged. (Figure) Discussion: Rectal MALT lymphoma is rare with unclear management options. Treatments of UC include watchful waiting, surgical resection, endoscopic mucosal resection, radiation, and/or chemotherapy. Helicobacter pylori infections, though strongly linked with gastric MALT lymphoma, have not been shown to be strongly correlated with rectal MALT lymphoma. Given that patients with UC have chronic UCassociated colonic inflammation, lymphoma is often difficult to distinguish visually during colonoscopy, frequently masked by ulcerations and pseudo-polyps. In cases like these, more definitive treatments such as surgical resection could therefore be warranted. Long-term follow-up data is sparse and definitive management remains a clinical conundrum, thus these patients require reliable long-term multidisciplinary close follow-up. (Figure Presented).

The [18F]FDG PET/CT is a crucial tool in the diagnostic process and monitoring of neoplastic diseases. Currently, during the global program of vaccination against COVID-19 and the possibility of axillary lymphadenopathy after this injection, the correct interpretation of PET/CT images is vitally significant and may create some difficulties. We present a case of increased uptake of [18F]FDG in an axillary lymph node in a PET/CT scan performed 2 days after the Pfizer BioNTech COVID-19 vaccine in a 48-year-old patient newly diagnosed with marginal zone B-cell lymphoma.Copyright © 2023 Via Medica.

The COVID-19 outbreak has fashioned to severe threat to each and every individual in social and economic aspects in the country. This required improved wisdom to know how it is different and dominant, to diagnose and determine effective vaccines to avoid the transmission of these deadly causative agents. From this review, the probable property of these deadly transmissible viruses is related to that of SARS-CoV-2 as a fright zone of viruses. It also provides some sparks about effective and accurate diagnosis and treatment strategies. ABSTRACT

Introduction: Advanced MZL is generally incurable, with periods of remission and relapse. Zanubrutinib (BGB-3111), a potent and highly specific next-generation Bruton tyrosine kinase (BTK) inhibitor, was approved in the US and Canada for R/R MZL based on the MAGNOLIA primary analysis (BGB- 3111-214;NCT03846427);here, the final MAGNOLIA analysis is presented. Method(s): This was a phase 2, multicenter, single-arm study of adult patients (pts) with R/R MZL (>=1 prior CD20-directed therapy). Zanubrutinib (160 mg twice daily) was given until disease progression or unacceptable toxicity. The primary endpoint was overall response rate (ORR) by independent review committee (IRC) per Lugano classification. Secondary endpoints were investigator-assessed ORR, duration of response (DOR), progression-free survival (PFS), overall survival (OS), and safety. Efficacy was assessed by positron emission tomography (PET)-based Lugano criteria for IRC-confirmed fluorodeoxyglucose (FDG)-avid disease at baseline;non-avid disease was assessed by computed tomography (CT)-based criteria. Result(s): As of May 4, 2022, 68 pts were treated (median age=70 y [range 37-95];>=75 y=27.9%). MZL subtypes included extranodal (38.2%), nodal (38.2%), splenic (17.6%), and unknown (5.9%). The median number of prior therapies was 2 (range 1-6);32.4% of pts had disease refractory to last therapy, most (89.7%) had prior chemoimmunotherapy, and 7 (10.3%) had rituximab monotherapy as their only prior treatment. Sixty-one pts (89.7%) had FDG-avid disease. After a median follow-up of 28.0 mos (range 1.6-32.9) and a median treatment duration of 24.2 mos (range 0.9-32.9), 66 pts were efficacy- evaluable. IRC-assessed ORR (complete response [CR]+partial response [PR]) was 68.2% (CR=25.8%). By subtype, (Figure Presented)(Figure Presented)ORR/CR rates were 64.0%/40.0% (extranodal), 76.0%/20.0% (nodal), 66.7%/8.3% (splenic), and 50.0%/25.0% (unknown). Median DOR, PFS, and OS were not reached. Over 70.0% of pts were alive or progression-free after 2 years (Figure). Sensitivity analysis using only CT-based criteria (n=66) showed an ORR of 66.7% and CR of 24.2%. The most common treatment-emergent AEs were bruising (23.5%), diarrhea (22.1%), and constipation (17.6%). Neutropenia (8.8%) and COVID-19 pneumonia (5.9%) were the most common Grade >=3 AEs. Five pts (7.4%) died due to unrelated AEs: COVID-19 pneumonia=2, acute myeloid leukemia=1, myocardial infarction=1, septic encephalopathy=1. Hypertension occurred in 3 pts (4.4%), atrial fibrillation and atrial flutter in 1 pt (1.5%) each;none led to treatment withdrawal. One pt (1.5%) had a Grade 3 gastrointestinal hemorrhage while receiving rivaroxaban. None of the pts required dose reduction. ABSTRACT

Small air ions have the ability to charge airborne particles, thereby increasing their accumulation on surfaces. Indoor air purification by applying ionization uses electrostatic particle deposition. Respiratory pathogens, including viruses and respiratory droplets carrying viruses or other pathogens, represent bioaerosols, whose particle size distributions contain increasingly larger proportion of fine and ultrafine particles, as the evaporation process proceeds. We have generated two model aerosols: the nebulized NaCl solution, resembling human saliva, and the cigarette smoke, having relatively low water content. We have conducted real life experiments of such surrogate aerosol particle deposition without ionization, using bipolar ionization, as well as using unipolar negative air ions. Particle number concentrations have been measured in the 10 nm–10 μm particle size range. The calculated deposition rates and aerosol particle half-life times were correlated with bioaerosol pathogens based on the core pathogen sizes. Bipolar ionizers emitting equal concentrations of positive and negative ions had low impact to the particle concentration decrease. Intense negative air ionization resulted in pronounced deposition rate increases, particularly in the particle size range of viruses including the SARS-CoV-2. The impact of negative air ionization was most pronounced in the same size range where the deposition rates without ionization were the lowest. Therefore, the results are very promising from the standpoint of air purification and bioaerosol pathogen removal, bearing in mind that the effect of ions will be most pronounced if the unipolar ion rich air stream is directed towards the breathing zone.

ABSTRACT

Abrasion causes coastal areas to have a very high level of dynamics and often has an impact on environmental damage, so it needs to be anticipated. In the last 2 years, the whole world has faced a crisis caused by the pandemic due to Corona Virus Disease (Covid-19). During the pandemic, the environment where coastal communities live is still threatened with abrasion. Some mangrove planting and rehabilitation activities had to be stopped to suppress the spread of the virus. The purpose of this study was to identify the role of community groups (Tourism Awareness Group) as a key factor in overcoming abrasion events during the Covid-19 pandemic at Bungin Beach. Interviews and direct observations were conducted to obtain survey data. Additional analyzes were then carried out through relevant journals and literature reviews. As a result, from 1997 to 2021 there has been abrasion on Bungin beach. The Tourism Awareness Group plays a role in post-pandemic recovery, especially in the economic sector. Through mangrove planting activities, not only to restore the economy of coastal communities during the pandemic but also to protect coastal ecosystems from the threat of abrasion on Bungin Beach.

ABSTRACT

This paper presents a Q-learning-based pending zone adjustment for received signal strength indicator (RSSI)-based proximity classification (QPZA). QPZA aims to improve the accuracy of RSSI-based proximity classification by adaptively adjusting the size of the pending zone, taking into account changes in the surrounding environment. The pending zone refers to an area in which the previous result of proximity classification is maintained and is expressed as a near boundary and a far boundary. QPZA uses Q-learning to expand the size of the pending zone when the noise level increases and reduce it otherwise. Specifically, it calculates the noise level using the estimation error of a device deployed at a specific location. Then, QPZA adjusts the near boundary and far boundary separately by inputting the noise level into the near and far boundary adjusters, consisting of the Q-learning agent and reward calculator. The Q-learning agent determines the next boundary using the Q-table, and the reward calculator calculates the reward using the noise level. QPZA updates the Q-table of the Q-learning agent using the reward. To evaluate the performance of QPZA, we conducted an experimental implementation and compared the accuracy of QPZA with that of the existing approach. The results showed that QPZA achieves 11.69% higher accuracy compared to the existing approach, on average.

ABSTRACT

In recent years, James Clifford's (1997) notion of museums as contact zones seems to have finally arrived in Germany's ethnographic museums. However, many of the newly established collaborative projects faced setbacks in 2020, as working with others on collections became impossible because of the COVID-19 pandemic. Normally not known for their digital progressiveness, museums suddenly embraced communication software and online platforms in order to stay in touch with their project partners. Apparently, the pandemic accelerated the digitization of the museum. In this article, we examine the implications of digitalization for German ethnographic museums as a contact zone. Through our conversation with Andrea Scholz since 2021 curator for transcultural collaboration at the Ethnological Museum of Berlin and an expert in digital collaboration we explore what epistemological effects an expansion into the digital might have on the collaborative production of knowledge for which museums strive today. After situating the recent push for both digitalization and collaboration within the growing engagement with the colonial histories of ethnographic museums, we consider what digitalization means for collaborative projects and the unequal relations of power that continue to underlie them. Revisiting Clifford's concept, Robin Boast (2011) reminds us that, as long as the authority remains with the museums, the contact zone is an asymmetrical space. Reconstructing our conversation, we discuss how digital exchange and networking might facilitate the permanent inclusion of divergent postcolonial perspectives that might help to overcome these unequal power relations.

ABSTRACT

The aim of the article is to present the role of economic activity zones in the development of local urban centres and their surroundings. The main area of research was the area of the town and commune of Wieliczka, with particular emphasis on the Wieliczka Economic Activity Zone (WEAZ). As part of the analysis, attention was focused primarily on the impact of enterprises on the labour market and the income of the commune. Based on the existing literature studies, the scale of multiplier effects resulting from the development of enterprises in WEAZ was also estimated. The presentation of the impact of companies located in the Wieliczka Economic Activity Zone on the socio-economic development of the commune area made it possible to determine the scale of benefits resulting from investing public funds in the creation of this type of investment zones. Analysing the role of WEAZ in the development of the local economy, the authors identified the relationship between the inflow of investments and a permanent increase in demand on the local labour market. This also concerned the resilience of newly created jobs in the context of the SARS-CoV-2 pandemic.

ABSTRACT

This paper discusses the design of a net-zero single-family house in Chattanooga, Tennessee, which was proposed for the 2022 Solar Decathlon Design Challenge entry. The site was in a 100-year flood zone and registered as a national and local historic district. The interdisciplinary team consisted of University of Tennessee Chattanooga (UTC) students from the interior architecture and engineering departments who embraced the challenges of the local climate and the restrictions of the historic district guidelines. This entry, Moc House, focused on the integration of passive design and active HVAC systems including photovoltaic systems in the process. In responding to recent housing market changes in Chattanooga due to the COVID-19 pandemic, a young professional couple in their 30s who have two children was identified as the target market which guided the space planning. Market analysis, including ROI, was conducted in addition to construction cost calculations. Building materials were selected to enhance the durability and resilience of the house structure. Finally, energy analysis was conducted to check the project's Home Energy Rating using Ekotrope and found it less than zero. © 2023, College Publishing. All rights reserved.

ABSTRACT

The COVID-19 pandemic had enormous health, economic and political consequences. In this article we argue that, after some initial confusion, the European Commission and European Central Bank, contrary to what had happened with the Euro-zone crisis, have profoundly altered their policies to address the dire consequences of the pandemic. The analysis is predicated around the case of Italy, which was the first country to face the spreading of the virus among its population as well as one of the most seriously hit in both health and economic terms.
